If you can not make your appointment, you must cancel or change it 24 hours in advance.
Two missed appointments or cancellations on the same day will need to pay no-show fees of $50.
If you have special circumstances regarding a missed appointment, you may discuss it with the manager.

MSP does not pay for the forms to be completed. Therefore, it is the patient’s responsibility to pay.

Your test result can not be provided to you otherwise it has been reviewed by your health care provider.
